| | You will be eligible for your cashback 180 days after your insurance plan start date, provided you haven’t cancelled your insurance plan and your insurance premium is paid in full or payments are fully up to date if paying by Direct Debit. |
| | If you have previously held and/or cancelled a Vitality Health Insurance plan within the last 12 months, you will not be eligible for this offer. |
| | Vitality reserves the right to cancel your Vitality Health Insurance plan or your eligibility for cashback should we discover or have a reasonable suspicion that the terms and conditions of this offer are being abused. |
